Responsibilities

The HUD Services Coordinator acts as a facilitator to maintain residents’ independent living status, to assess the individual needs of residents and work with them to establish and maintain linkages to needed supportive services or medical services provided by private practitioners within the general community. The Services Coordinator may assess service needs and determine eligibility for public services. This position maintains regular contact with identified frail residents monitoring the delivery of supportive services and advocating for changes in services desired or required by the resident. The HUD Services Coordinator acts as a liaison between management and residents, and provides information and assistance to the Site Leader in the operation of each building in accordance with HUD guidelines.

Qualifications
• BA/BS in social work, gerontology, psychology, or counseling is required. Licensure or certification in the listed fields is helpful.
• Two to three years of experience in social service delivery with senior citizens and non-elderly disabled. Some supervisory or management experience may be desirable.
• Demonstrated working knowledge of and training in supportive services and other resources for senior citizens and non-elderly disabled in the area served by the project.
• Training in lifecycle issues of older persons and families, as well as physical and mental impairments common to the aging process. Demonstrated ability to advocate, organize, problem-solve and provide results for the elderly and disabled population. Ability to empower residents to make personal decisions, increasing their self-esteem and independence.
• Training in HUD subsidy programs.
• Demonstrated ability to advocate, organize, problem-solve and provide results for the elderly and disabled served.
• Ability to create a services "team" in which the service coordinator supervises one or more aides, the coordinator should have appropriate professional staff experience and prior supervisory or management experience.
• Demonstrated compatibility with PHS's mission and operating philosophies.
• Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.
