BayCare is currently in search of our newest Certified Diabetes Education Coordinator who is passionate about providing outstanding service to our patients. We are looking for an individual seeking a career opportunity with one of the largest employers within the Tampa Bay area.
Responsibilities:
• Responsible for planning, implementing and evaluating diabetes self-management education (DSMT/E), its related services & other education programs, including diabetes prevention.
• Ensures systematic and coordinated delivery of all services.
• Performs Certified Diabetes Educator functions, providing patient-centered diabetes self-management training, education and support to individuals/groups according to the Scope of Practice, Standards of Practice, and Standards of Prof Performance for Diabetes Educators (AADE), expert resource for people/providers.
• Involved in case & clinical management, healthcare consultancy with other providers, organizations, industry, public & professional education, public health & wellness promotion, research in diabetes management & education, ensures requirements set forth by the CMS accrediting process (ADA or AADE) are met.
• Participates on Diabetes Service Line and supports efforts to attain/maintain disease-specific certification by The Joint Commission.
• CDR required with Florida RD or BLS required with RN license.
• Required Experience includes 3 years as a Certified Diabetes Educator.
