The Aging Care Coordinator supports clients and their families by providing comprehensive assessments, developing care plans, monitoring ongoing care, managing the Therapeutic Companion, communicating with and coaching the family, and coordinating the interdisciplinary team.

The Aging Care Coordinator is the point person for the family and will act as the liaison between the client, family, and providers to ensure effective care. Aging Care Coordinator will meet clients in the community, remotely, or in the home setting to perform actions necessary to meet their needs and goals.

Job Responsibilities:
• Completing initial evaluations and comprehensive assessments of the client and their environment that are done both in-person and virtual
• Conducting psycho-social evaluations and assessing cognitive abilities and mood
• Reviewing clinical history and contacting providers
• Developing a care plan with the client and identified supports
• Maintaining communication with client contacts, including family, physicians, attorneys, financial offices, and other care supports
• Supervising Therapeutic Companions and working collaboratively with other OPG team members
• Documenting all client interactions, outcomes, and progress toward goals
• Providing consulting services to the client and family members around aging
• Conducting family meetings to improve family dynamics
• Providing coaching services to identified individuals
• Communicating with outside treatment providers and coordinating care
• Traveling throughout the area for evaluations, home visits, and appointments
• Advocating for clients based on individual medical, financial, and social needs
• Having a basic understanding of the differences of Medicare, Medicaid, Private Insurance, and Long-Term Care policies
• Having a basic understanding of care management
• Assisting clients in moving into senior living communities
• Participating in company supervision and completing required company education materials

Additional Responsibilities:
• Emergency support, on-call availability, and client transportation, as needed
• Additional duties will be assigned as deemed appropriate

Qualifications:
• RN, LSW, LCSW, LICSW, OT, or PT in good standing preferred; Case Manager or Care Advocacy in the elder field with 5+ years of experience also considered
• 4-5 years of experience in care management, home care, acute care, and/or a rehabilitation setting
• Experience with elder care
• Experience in behavioral health preferred
• Experience working closely with families & professional providers
• Excellent written and oral communication skills

14. Enters individual data in the SAMS database for maintenance of records; searches computer database for individual information as needed.

Clearances:
• Must work in conjunction with employer to apply for Act 34 PA State Police Criminal History, Act 151 PA Child Abuse and FBI clearances prior to employment. Clearances currently possessed that are less than 4 years old will be accepted until new clearances are available. All clearances must reflect acceptable results.
• Must not appear on Preclusion Lists as defined by “Pennsylvania’s Medical Assistance Bulletin 99-11-05 Provider Screening of Employees and Contractors for Exclusion from Participating in Federal Healthcare Programs.”
• Must successfully complete pre-employment drug screening.

An Aging Care Manager 3 with the County of Northampton provides care management services for vulnerable older adults who are referred for protective services, as part of the County Area Agency on Aging. The Aging Care Manager 3 conducts investigations according to PA Older Adult Protective Services Act, and advocates for them; care management begins with receiving a referral/report of need for protective services and ends when the consumer no longer meets definition of an older adult in need of protective services; makes referrals and authorizes Northampton County services for consumer care.
