This position will be filled under the Pathways Internship Program. This Program allows students to gain paid work experience while pursuing a degree, diploma or certificate at a qualifying institution. Under the Pathways Program, students who successfully complete all work, study, qualifications, and other eligibility requirements may be offered a permanent position work our agency upon graduation or completion of a qualifying certificate.
At the Agency's discretion, students hired under the Pathways Program may be non-competitively converted to a term, career, or career-conditional appointment with a possible Full Performance Level (FPL) of GS-05, GS-07, GS-09, GS-11, GS-12, or GS-13 (FPL varies based on hiring department).

• Researches and investigates census data to include the identification and collection of missing information, modification of incorrect information and proper documentation.
• Develops and maintains good working relationships with facility clinical contacts.
• Contacts assigned hospitals to review and collect updated patient information, review and update census data and notes.
• Documents all clinical updates including discharges and follow up in Brainstream daily.
• Communicates escalation issues to the department director for resolution.
• Coordinates patient discharge plans to reduce readmission risk.
• Completes “warm transition” of patients / processes to internal management teams.
• Navigate multiple EHR/EMR/practice management systems.
• Maintain beneficiary’s privacy, confidentiality and safety, advocacy, adherence to ethical, legal, and accreditation/regulatory standards during this process.
• Participate in professional development activities to include current principles, procedures, and knowledge domains of case management based on nationally recognized standards and licensure requirements.
• Performs all duties within the scope of his/her licensure.
• Other duties as requested.
Knowledge, Skills and Competencies
• Demonstrated ability to adapt and be flexible in an environment that moves at a rapid pace, where change is common and frequent.
• Solid working knowledge of Excel with proficient skills in MS Word, PowerPoint, and Outlook
• Ability to manage multiple projects and conflicting priorities.
• Excellent time management skills.
• Strong Problem solving and critical thinking skills.
• Excellent communication skills; verbal and written, and organizational skills
Education and Experience
• Active RN, LPN, LMSW, LCSW or comparable clinical license/certification.
• 2-4 years experience in in-patient or out-patient patient care, case management, admissions, or similar care experience
• Experience working with Medicare and Medicare Advantage plans
Physical Requirements
• Prolonged periods of sitting at a desk and working on a computer.
