SUMMARY

1. Diabetes Education and Care (60%)

a. Develops, assesses, implements and evaluates education plan for patients with diabetes
b. Performs diabetes-specific physical, educational and psychosocial assessments
c. Facilitates patients' highest level of functioning through the application of culturally relevant interventions
d. Delivers all aspects of the educational plan to patients
e. Assesses and recommends appropriate interventions for clients who experience difficulties in managing condition, including basic insulin dose adjustment. Makes medication changes or follows medication adjustment protocols or makes necessary recommendation to primary care providers.
f. Develops teaching materials that support patient learning needs
g. Participates as multidisciplinary team member to enhance the delivery of diabetic patient care

2. Team Collaboration in Diabetes Education Services (25%)

a. Teaches classes about diabetes management basics and sick day care for caregivers and nursing staff
b. Glycemic surveillance: Reviews daily reports on inpatients that have significant glycemic abnormalities and works with inpatient teams to see what insulin adjustments are needed or if endocrine consultation is needed
c. Assists with discharge process for diabetic patients.
d. Creates educational materials for patients undergoing surgical procedures and/or imaging, including patients who use insulin pumps
e. Creates educational modules for providers and other members of institutional staff about management of glycemic abnormalities, including information about glucose monitoring, basal/bolus insulin therapy and appropriate dosing
f. Collaborates with Patient Education Department to develop and monitor educational resources to meet the needs of the diabetic population

3.Strategic Program Development (10%)
a. Functions as a multidisciplinary team member, working with physicians to enhance the delivery of diabetic patient care
b. Participates in educational and clinical research to provide optimal services to patient with diabetes
c. Trains, coordinates, and develops staff to assist in the delivery of diabetic teaching to patients and families
d. Assist in pursuing ADA certification for diabetes self-management program, if determined feasible by department or institutional management

The Position

Responsible for working with health system accounts and networks to execute the Diabetes Education Program strategy. Interacting with health system KOLs, providers, quality, care coordinators, advanced practice providers, and the like to identify their poorly controlled diabetes populations and support their efforts to deliver better care through diabetes education and training following ADA Standards of Care and relevant treatment guidelines. This position will report through commercial health system leadership and is part of the account teams focused on establishing Novo Nordisk as the leader in diabetes care and increasing the demand for the Novo Nordisk Inc., (NNI) diabetes portfolio of products.

Relationships

Externally, the Diabetes Educator maintains relationships with physicians, advanced practice providers, pharmacists, nurses, patients and other key personnel within targeted health system accounts. Additionally, the diabetes educator will interact with key health system departments including quality, care management, population health, diabetes treatment centres, and other key departments focused on improving diabetes management across the system population.

Internally, the Diabetes Educator reports to the Health Systems Diabetes Educator Manager (HSDEM). The Diabetes Educator primarily aligns and liaises with the Health System Account Manager (HSAM) and/or other internal key health system account management as part of NNIs coordinated approach to health system engagement. Other key internal stakeholders include sales, market access, marketing, and medical personnel. This position is part of NNIs commercial organization and is responsible for key commercial performance and deliverables.

Essential Functions
• Work with health systems accounts to identify “at risk” patient populations and utilize diabetes education following the ADA Standards of Care and other relevant treatment guidelines to appropriately recommend the most appropriate Novo Nordisk’s products and solutions approved for the customer’s needs
• Leverage PHM priorities/value-based models to change health system and provider behavior
• Maximizes opportunities with NNI brand strategies, pull-through and support programs to improve diabetes care
