JOB SUMMARY:
The Care Coordinator will support the Infectious Disease clinic and the Palliative care ambulatory clinic. The Coordinator, in collaboration with Infectious Disease and Palliative Care specialty medical providers, performs complex case management and direct patient care to assigned patients. This includes point of care procedures, management of medical devices, ongoing patient, family and staff education and coordination of care with outside community agencies. The coordinator also triages and manages patient requests and needs via telephone triage and patient messaging via EPIC. The coordinator also partners and supports continued growth of medical assistants to deliver well rounded care. The with Infectious Disease and Palliative Care specialty divisions, both care for patients with general and complex diagnoses and symptoms. The Coordinator facilitates effective functionality of clinic operations, actively participates in Quality Management and acts as a provider extender within his/her scope of practice. The Care Coordinator performs additional team functions as required to maintain effective customer service. The coordinator role in Gastroenterology has a blend of hand on procedures, complex patient care coordination, education on disease management, triage, and in clinic support. We are looking for someone that is autonomous, collaborative, supportive of teams, educator, customer focused, and solution driven. The coordinator would be joining a large division with a collaborative partnership with Providers and ancillary support staff as well as an engaged and dynamic team.

MINIMUM QUALIFICATIONS:
• Associate's Degree
• Area of Study in a Field Related to the Position (GI)
• Two years of experience
• California RN License
• BLS for Healthcare Providers, issued by AHA

PREFERRED QUALIFICATIONS:
• Five years of experience
• Bachelor's Degree in Nursing
• Ped Adv Life Support Prov
• Bilingual – Spanish
• Ambulatory care experience
• Pediatrics experience

The current salary range for this position is $51.94 to $89.38

Shift: 10 hours shifts-7a-5p, rotating call coverage(1 night/week, 1 weekend every 8 weeks)

• You will be responsible for delivering patient care through the nursing process of assessment, diagnosis, planning, implementation and evaluation.

The Triage RN is primarily responsible for assessing problems over the phone and directing healthcare services after business hours. Triage RN uses community resources to direct services to meet the needs of individuals and families within their homes and communities. The Triage RN must possess and use good clinical judgment, careful listening, critical thinking, assessment skills, and triage experience.

Qualifications:
• Graduate of an accredited school of nursing.
• Current state nursing license.
• One (1) to two (2) years of recent acute care, triage and/or home health care experience
• Recent triage experience
• Experience using technology including: Microsoft Office, Smart Phone, Calendaring, Clinical Software platforms (EMR), Wi-Fi, Printers, and Internet Browsing
• Proficient in understanding and use of multiple Clinical Software platforms (EMR).
• Proficient in managing multiple phone lines and a steady influx of calls.
• Ability to follow written instructions and type a minimum of 20 words a minute.
