firsthand Guide

We named our company firsthand because we believe that lived, or “first-hand,” experience is incredibly valuable to gain trust and build relationships. The firsthand Guide is a critical role in our peer-led model, responsible for outreach and engagement of individuals living with serious mental illness to achieve and maintain behavioral and physical health and wellness. Trust is an essential element in successfully engaging people who have withdrawn from, or never received, services and supports that can improve their quality of life.

Focusing on support and services that promote whole health and reinforce autonomy, the firsthand Guide will help empower individuals to participate in their own care. firsthand Guides are empathetic, supportive, and highly trained and experienced individuals with first-hand knowledge of living with a psychiatric and/or substance use disorder, and are familiar with the intricacies of navigating a complex system of healthcare.

The firsthand guide assists other members of the firsthand Care Team such as the Health Guide (e.g. Nurse Practitioner) in addressing the individual’s health needs (not including diagnosis or choosing a specific treatment plan), and the Community Resources Guide in meeting SDOH needs and with disability applications.

As firsthand Guide, you will:
• Perform community outreach to individuals with serious mental illness, including visiting their homes and other locations within the local community where they spend time, such as clubhouses, drop-in centers, homeless shelters, hospitals, and emergency departments
• Build trust and develop relationships with individuals living with SMI, by first identifying and addressing immediate needs
• Empower individuals to identify and pursue their personal recovery goals
• Model and teach safe and healthy coping strategies
• Serve as primary point-of-contact for individuals, facilitating ongoing engagement
• Apply advanced engagement techniques; on-the-job training will be provided as needed
• Partner with individuals to facilitate engagement in supportive services to help meet self-directed wellness goals
• Identify social determinants of health that may be able to be addressed by firsthand
• Facilitate warm hand-offs to other team members for assistance in navigating benefits and resources
• Assist with gathering information required for obtaining benefits and services
• Address transportation barriers by assisting in accessing transportation benefits, navigating public transportation, and providing transportation as needed
• Engage in care coordination to assist individuals in accessing appropriate mental and physical health services, and advocate for the individual’s personal goals with providers
• Provide support via activities such as accompanying the individual to appointments
• Address immediate follow-up needs identified by care team providers such as hospital post-discharge follow-up and support

The Community Resources Guide (CRG) is responsible for linking newly engaged individuals with serious mental illness to community-based supportive services in order to address social determinants of health and support their overall wellness. The CRG will do this by working with peer recovery specialists (firsthand Guides) to empower individuals to identify and achieve their own recovery goals. Supportive services may include rental/utility assistance, access to food or pantries, Social Security benefits, primary care, or community behavioral health services.

The CRG must be able to work in the community, meeting individuals where they are, and be comfortable working in non-traditional settings and unstructured environments. Experience assisting individuals with Social Security benefits enrollment, including SSI/SSDI Outreach, Access, and Recovery (SOAR) certification, is highly preferred.

As Community Resources Guide, you will:
• Perform social and behavioral health assessments to support individuals living with serious mental illness in identifying barriers to wellness.
• Build personalized initial care plans to address social needs.
• Link individuals to community-based resources and supportive services including SSA benefits, community programs, housing, food, etc.
• Complete interviews with individuals, gather medical records, and other information to complete SSI/SSDI applications.
• Engage in care coordination as part of a multidisciplinary team of peers, social workers, and clinicians to assist individuals in accessing appropriate services and support.

This position will provide the opportunity to help make a difference in various communities engaged by firsthand's missions and goals. The Credentialing Associate will support a robust and growing professional community and serve as a focal point for the credentialing process to internal and external parties.

As a PRN- Contract Credentialing Associate you will:
• Research, analyze, evaluate, distribute, receive, and review credentialing applications, ensuring all relevant information has been provided and or submitted.
• Ensure credentialing and recredentialing applications are processed, tracked and monitored for adherence to timelines and other processing requirements.
• Enter demographic and credentialing information into credentialing software and other logs in a timely and accurate manner to facilitate, monitor and document credentialing efforts.
• Learn, understand, maintain and ensure that Industry standards, regulations and firsthand's credentialing policies and procedures are followed,

This role will help support our colleagues as they cope with particularly trying situations (could include the death of an individual we've supported, a trauma sustained in the line of work, or another difficult situation that affects part of or a whole Community team).

We estimate that this PRN Contract role will vary in hours from month to month based on need. This is not a FT position.

On an as-needed basis, the part-time care provider will:
• Provide critical incident care support to firsthand Community teams in Florida, Ohio or Tennessee
• Provide support either virtually (telehealth) or in-person, as business need permits
• Gather context and requirements from Community leadership or the firsthand People team

Requirements
• Deep empathy for the work of the firsthand Community teams and the individuals to whom they support

The Health Guide is a licensed Nurse Practitioner or Physician Assistant who leads the medical aspects of team-based care for individuals with serious mental illness that firsthand serves. The Health Guide is responsible for whole person assessment and management of our vulnerable population, collaborating closely with community primary care and behavioral health providers, as well as an internal team of firsthand Guides (peers) and Community Resources Guides (social services personnel).

The Health Guide assists other members of the firsthand Care Team such as the firsthand Guide (peer specialist) in supporting individuals achieving recovery, and the Community Resources Guide in meeting SDOH needs and with disability applications.

As a Health Guide, you will:
• Perform a whole-person clinical assessment, including:
• Assessing both behavioral and physical health conditions with a thorough history and physical examination
• Capturing conditions not yet diagnosed, ordering additional studies where indicated to confirm or rule out the diagnosis
• Collaborating with a team of support including medical records and clinical documentation specialists, certified coders, and medical assistants
• Conducting visits in the in the office, virtually, in the field, or in an individual’s home when needed
• Making individualized recommendations and/or referrals for programs/care provided by firsthand and/or outside providers, based on the assessed need
• Supporting individuals in developing and exercising self management skills and person-centered care around their individual goals and preferences via motivational interviewing and holistic care planning
• Organizing the information in a manner that supports appropriate clinical documentation and successful disability applications (if appropriate)
• Lead the medical component of the team-based care provided by firsthand, including: Training and advising roles like Medical Assistants, and Peer Recovery Specialists on behavioral, clinical, and social health priorities

• Leading care and disease management - identifying actionable care gaps
• Communicating and coordinating with individuals’ care providers, primarily at CMHCs, FQHCs, and hospitals
• Providing direct patient care when needs cannot be met by existing community providers

• Collaborate with other team members to provide integrated care

Our initial firsthand Guides are empathetic, supportive, and experienced individuals with first-hand knowledge of living with a psychiatric and/or substance use disorder, and are familiar with the intricacies of navigating a complex system of healthcare. Now we are now excited to expand our firsthand Guide role to people with lived experience of caring for people living with SMI and who have trained as Certified Family Peer Supporter.

As a Family Peer Supporter (firsthand Guide) you will:
• Perform community outreach to individuals with serious mental illness, including visiting their homes and other locations within the local community where they spend time, such as clubhouses, drop-in centers, homeless shelters, hospitals, and emergency departments.
• Build trust and develop relationships with individuals living with SMI, by first identifying and addressing immediate needs.
• Draw on your own experience of supporting family members with emotional or behavioral issues as you work with the parents/caregivers to discover and support their loved ones values and priorities.
• Promote services that are family driven, youth guided, trauma-informed, and culturally competent and provide guidance, advocacy, and camaraderie for parents and caregivers of family members living with SMI and/ or SUD
• Engage in care coordination to assist families in accessing appropriate mental and physical health services, and advocate for the individual’s personal goals with providers.
• Provide coverage for after-hours calls on a rotating schedule.

You will be a good fit if you have:
• Empathy, compassion, and approachability
• Been or are currently the caregiver of an individual with behavioral health challenges (mental health/substance use disorder); and who has navigated service systems for at least one year on behalf of the individual.
• Excellent listening, communication, and interpersonal skills
• Comfort working as part of a multidisciplinary team
• Good computer skills

The experience you bring to this role includes:
• A high school diploma or GED
• Certified Family Peer Supporter (CFPS) approved by OhioMHAS
• Knowledge of the OhioMHAS Family Peer Recovery Services Code of Ethics
• A valid drivers license and clean driving record

$18 - $18 an hour
This is a full-time, non-exempt, hourly position reporting to firsthand's Guide Team Lead. firsthand cares deeply about ensuring pay equity with every team member who joins. Commensurate on candidate experience, the expected hourly rate for this role is $18/hr.

As Analytics Engineer, you will:
• Build lasting data modeling solutions, including automating analyses and authoring pipelines via SQL-based ETL frameworks
• Develop data models and schemas that enable performant and intuitive analysis in a Data Warehouse setting
• Communicate and collaborate with different stakeholders like Data Scientists, our Product team and clinicians to produce requirements and detailed designs
• Help operations and clinical managers build data-driven processes
• Invest in end-user data literacy through hands-on training with folks around the company
• Enable teams to self serve data analytics needs by transforming data into useful formats and documenting data models using tools like Secoda
• Validate incoming data to ensure usability, map it to platform specifications and perform verification

You will be a good fit if you have:
• Experience with clinical medical records such as problem lists, medications and notes
• Experience with data modeling such as transforming data interchange formats like FHIR into an analytics schema
• Experienced SQL userExperience building reports and dashboards using business intelligence tools
• You are a team player and are interested in working at a small, rapidly-changing, fast-paced startup environment

The experience you bring to this role includes:
• You have 2+ years of relevant experience in healthcare analytics. Familiarity with dbt required

The Role

Firsthand is looking to hire a Sales Director as a foundational member of the company's sales team. The Sales Director is responsible for building success by identifying, qualifying and selling agency & advertiser prospects on the value of Firsthand. The ideal candidate will be located in the New York metropolitan area.

This senior seller will be responsible for supporting new and existing business, creating an actionable plan of execution for account prioritization, and taking Firsthand’s AI agent platform and solutions to market. The best candidates bring relationships with them to the table, and they are comfortable presenting at both a tactical and strategic level; all the while evangelizing, educating, and influencing agency key decision-makers.

Responsibilities
• Establish and grow relationships with key agencies and advertisers
• Assess potential business deals, through RFP responses and pursue proactive opportunities via strategic partnerships
• Network at industry-relevant functions to increase Firsthand’s visibility and connect with new clients
• Maintain proper adherence to company standards for data entry/utilization through Firsthand's CRM
• Effectively forecast monthly, quarterly, and annual sales
• Develop proficiency in Firsthand products/technology, and understand how they fit within the advertising ecosystem
• Collaborate closely with company leadership on sales, leads, and the status of prospective projects
