Responsibilities

Summary

The Mental Health Case Manager is responsible for providing the following mental health services to GEO Participants: Initial Screening, inter-agency service coordination, supportive counseling, case management, group facilitating as needed, suicide risk assessment, crisis intervention, discharge planning and to support participants in securing housing, financial support, educational/vocational programming, and medical care.

Primary Duties and Responsibilities
• The Mental Health Case Manager conducts and/or participates in mental health interviews, multi-disciplinary treatment team meetings and screenings, to determine appropriate mental health services.
• Provides onsite, supportive mental health services including individual and group counseling.
• The Mental Health Case Manager provides case management services as required to ensure continuity of ongoing service delivery.
• Prepares all related mental health documentation in accordance with applicable organizational and state standards in a clear, thorough, and timely manner.
• The Mental Health Case Manager responds to psychological emergencies as needed.
• Works with local community providers to help address participant housing needs, and primary medical care/physical benefits.
• The Mental Health Case Manager assists participants with social, educational, vocational, and other support methods as needed to support their financial needs and general well-being.
• Attends multi-disciplinary team meetings conducted by community providers/agencies/partners regarding participants progress and compliance, other professional meetings and represent the program to the community as needed.
• The Mental Health Case Manager exercises sound judgement, demonstrate initiative and maintain confidentiality in accordance with established policies and procedures, which comply with legal and administrative mandates.
• Performs duties in accordance with applicable professional ethics, state quality assurance standards.
• Makes collateral contacts with participants significant others, to promote implementation of the service plan and optimal community adjustment.
• Conducts multi-disciplinary discharge planning to ensure ongoing supportive services for participant.
• Performs other duties as assigned.

Qualifications

Minimum Requirements
• High school diploma or its equivalent and a minimum of two (2) or more years of full-time experience in a mental health related field such as Psychology, Counseling, or Social Work with the criminal justice population.
Bachelor's Degree is preferred.
• Two (2) years of experience providing delivery and/or coordination of mental health services.
• Ability to work in crisis situations in a professional and mature manner.
• Strong counseling and motivational skills.
• Must be able to work individually and in a team environment.
• Above average verbal and writing skills.
• Must be able to develop clear and concise reports in a timely manner.
• Ability to work with computers and the necessary software typically used by the department.
