Qualifications:

Duties and Responsibilities:

Reporting directly to the Sr. Advisor to the President for Inclusive Excellence and Equity Innovation Officer, the HSI Community Grant Analyst (HSI CGA) will be responsible for supporting the implementation of project activities for the CSUN HSI Community Grant awardees on behalf of the Global HSI Equity Innovation Hub and as directed by the grant Principal Investigator (PI), in coordination with key external project partners and the CSUN Community. The HSI CGA broadly supports the planning, implementation, administration, and evaluation of the HSI Community Grants program. Coordinates the day-to-day administration of the projects, including activities coordination, expenditure processing, budget analysis, coordination of events, reporting, compliance, and other related operational functions to meet programmatic goals. Coordinates and regularly liaises with individuals at all levels within the organization, including HSI Community Grants PIs, college administrative staff and leadership, the Office of the President, and the Global HSI Equity Innovation Hub leadership team. Coordinates data collection across all projects and complies with regular updates on the completion of the project objectives, key milestones, and reporting. Collects, compiles, and reviews needed budgetary and data metrics to assist with impact reporting to ensure timely compliance with grant reporting.
• Provides overall support for the day-to-day implementation and coordination of HSI Community Grant Project awardees. Provides lead work direction and coordinates with other project personnel, including PI's and the Global HSI Equity Innovation Hub staff. Broadly works to ensure that all components of the project, project activities, and partner sites function in alignment with the goals and objectives of the project. Support requires independent judgment to make decisions, interpret, and apply a wide range of policies and procedures related to processing required documentation for HSI Community Grant projects. Supports daily project activities and a range of complex administrative and operational functions across multiple projects to meet programmatic goals. These functions include acting as an internal and external resource on expenditure processing policy and procedural matters, coordinating projects involving the management of external partnerships, acts as a program representative to outside entities (i.e., Governmental, educational, community, and industry). Provides support with project scheduling, meeting coordination, project-related travel, and other tasks. Provide support to HSI Community Grant PIs with overall coordination.
• Provides support with expenditure processing and budget tracking and analysis, coordinating across multiple units and projects. Coordinates event planning and coordination of onsite and off-site programming, evenings and weekends, including the end-of-the-semester and end-of-year presentations of projects and research and professional development and training events. Supports the project by coordinating sessions such as hands-on activities, materials, idea exploration, and overall family engagement in a way that fosters and increases a sense of inclusion and belonging for program participants, partners, and other stakeholders.
• In coordination with PI and the Global HSI Equity Innovation staff, we will ensure that project-related activities have the appropriate resources, including resources for community members and participants, while ensuring compliance with applicable regulations or requirements governing the program. Conducts program and policy research and analysis as needed. Collects, analyzes, and summarizes reviews budget and programming data needed for ongoing reporting projects. Works to complete and submit all required reports in a timely manner.
• Participates in professional development and serves on relevant committees to further understanding of the project.
• Performs other duties as assigned.

Qualifications:
• Four years of full-time, progressively responsible administrative work experience as a secretary, executive secretary, staff assistant, other clerical and administrative support positions, or a combination of education and experience which demonstrates that the applicant has acquired the knowledge and abilities listed.

HSI is looking for a Patient Navigator to join our team. The Patient Navigator is responsible for ensuring that patient-centered care is delivered to HSI patients through regular and timely scheduling, interviewing (telephonic or face-to-face), clinical data gathering for chronic care management, and/or transitional care management in coordination with the patient care team.

ESSENTIAL FUNCTIONS:
• Act as a guardian of high-quality medical data and demonstrate concrete knowledge of state and federal guidelines for the Health Insurance Portability and Accountability Act (HIPAA) and other privacy standards.
• Work with providers’ clinical site of care as assigned to support the staff with care coordination of various patient population.
• May perform the duties of a case manager to assist patients in accessing available health care, social service, and related systems which contribute to physical and emotional health wellness of the patient.
• Assist with management of chronic care of Insured patients.
• Assist patients in understanding their treatment options.
• Serve as an essential link between patients and all care providers.
• Facilitate appointments for physician consults/referrals and support services within established HSI protocol and procedures.
• Acts as a patient advocate and presents their needs and preferences for care when supporting the procurement of services.
• Coordinates treatment programs by setting schedules and routines and follows up on services being provided.
• Monitors cases by verifying the patients' attendance with appointments and evaluating response
• Follow patients throughout the care continuum, including inpatient hospital admissions, as well as collaboration with hospital care management at the time of discharge.
• Assist Quality Improvement Nurse and Clinical staff with collecting data, tracking outcomes, and supporting HSI’s strategic planning processes.
• Submit monthly referral report to the Patient Navigator Coordinator, Director of Case Management/Referrals, and Chief Clinical Officer (CCO).
• Charts health data information as appropriate in medical records.
• Maintains departmental and patient files and records in accordance with prescribed administrative, medical, and work standards.
• Assist with outreach efforts to establish and maintain positive working relationships with key customers (physicians, office staff, nurses, ancillary staff, and case management staff).
• Assist with training of new HSI staff and providers as needed.
• All other duties as assigned by supervisor.

PHYSICAL REQUIREMENTS:

Combination of sitting, standing, walking, bending, lifting, carrying, speaking and listening on a regular basis.

EDUCATION, TRAINING AND EXPERIENCE:
• Associate degree in related field or Certified Medical Assistant required.
• Two (2) years of recent consecutive work experience as a Certified Medical Assistant or related experience.
• Possess knowledge of medical terminology, interpreting orders and developing a plan of managed care in accordance with physician request.
• Proven experience in the health care setting successfully performing care management and coordination of care duties.
• Knowledge of Electronic Medical Records (EMR).
