Reviews available patient information related to the case, including home visits, to determine hospice care needs. Assigns appropriate hospice personnel to case as needed. Supervises RN’s, LPN’s and Home Health Aides on all aspects of patients care within Hospice. Reviews and evaluates each case through a variety of means such as visits, conferences, record review and the services provided by clinicians.

The Clinical Supervisor will need to oversee the assessment of the patients' needs and make the appropriate determination of required actions within the context of the patients Interdisciplinary Plan of Care and Hospice Care mission, objectives, policies and procedures. The Clinical Supervisor will need to evaluate effectiveness of clinical directions, demonstrate knowledge and commitment to excellence in clinical practice, and coordinate with other members of the interdisciplinary team to ensure the patient and family's needs are met.
Requirements
Bachelor’s degree required, Master’s degree preferred. Graduate of an accredited school of nursing. This position requires strong leadership and clinical skills. Excellent communication (verbal and written) skills as well as strong decision-making, organizational and problem-solving skills are needed. Good computer skills are essential. Minimum 3-5 years relevant experience. Hospice experience required. Current New Jersey RN license. National Certification related to specialty area preferred.
Essential Functions Of The Job
Supervises hospice nurses, home health aides, and works with Intake to properly manage case referrals. Reviews available patient information related to the case, including home visits, to determine hospice care needs. Assigns appropriate hospice personnel to case as needed. Conferences with the Directors regarding any questions about an individual's eligibility for services and demonstrates working knowledge of regulatory practice guidelines.
Supervises RN’s, LPN’s and Home Health Aides on all aspects of patients care within Hospice. Reviews and evaluates each case through a variety of means such as visits, conferences, record review and the services provided by clinicians.
Reviews patient's medical diagnosis, prognosis, medications, procedures and clinical course
Assists clinicians in establishing immediate and long-term therapeutic goals, setting priorities, and in developing an appropriate plan of care.
Attends case conference meetings with staff to facilitate coordination of care and discussion of interdisciplinary group involvement.
Participates in record reviews according to QAPI guidelines and communicates findings and recommendations to Directors and staff as appropriate.
Assists the Quality and Education Coordinator in the education and orientation of all clinical employees.
Assists Directors and Quality & Education Coordinator in the planning, implementation and evaluation of in-service and continuing education programs. Assists in the formulating, revising, implementing, and evaluating organization policies, procedures, goals and objectives, both short and long range
Works collaboratively with the Directors to assure documentation is complete and consistent with care and reflects compliance requirements.
Responsible for the day-to-day management of all nursing and home health aide staff, including annual performance reviews as required and any performance management issues.
Participates in administrative on-call as required.
