Patient Care

Requirements:

• Relevant clinical competence in area of nursing practice assigned;Circulating experience either in a main Operating Room or a multi-disciplinary surgery center, or completion of a Periop 101 program preferred.
• Licensed as a registered nurse with current registration in New York State

Coordinates the work of Patient Representatives, students and volunteers, and assists in programmatic activities and budgeting. Responds to referrals from patients, families and caregivers and to crisis situations.
Roles & Responsibilities:
Provides pharmaceutical care to unit specific patients including the establishment of therapeutic endpoints and monitoring parameters.
Essential Duties and Responsibilities
1. Performs all duties associated with the Registered Pharmacist position within the scope of licensure.
2. Initiates and documents pharmacist interventions to change prescribing where required.
3. Participates in patient care rounds as appropriate.
4. Contributes to and participates in the departmental medication use evaluation plan, including but not limited to CMI (Clinical Management Initiatives).
5. Reviews current medical/pharmacy literature regularly for recent developments in disease state therapeutics as well as new product development.
6. Provides health professionals with drug information and/or references.
7. Prepares periodic information as newsletters for physicians, nurses and pharmacists regarding drug therapy and administration.
8. Provides inservice education seminars and programs to the pharmacy staff and other health care professionals.
9. Facilitates preparation of Pharmacy and Therapeutics Committee material.
10. Investigates and documents reports of Adverse Drug Reactions. Facilitates Adverse Drug Reaction Subcommittee meetings.
11. Completes all required documentation.

Education Requirements
Doctor of Pharmacy Degree from an accredited college of pharmacy.

Meets the requirements as set forth by the American Society of Hospital Pharmacists Residency Accreditation Standards. Familiarity with computers/systems helpful.
Licensing and Certification Requirements (if applicable)
Current New York State License and Registration as Pharmacist or eligibility for licensure.

Position Summary:
Valley Medical Group is seeking a full time, Licensed, Board-Certified/Board-Eligible Internal Medicine or Family Medicine Residency Trained Physician to join its growing practice. Physician will assure continuity of care through adequate scheduling and tracking of return visits, completion of treatment plans, and follow-up of chronic problems, abnormal tests, and “no-shows”. Provides appropriate patient education including clear explanation of treatment; the use and side effects of prescribed medications; health maintenance; and the use of appropriate educational “handouts” or other written instructions. Delegates clinical tasks appropriate to the level of training of each staff member. Provides an accounting of services rendered and accurate, specific diagnosis codes for each patient by completing appropriate billing/reimbursement documentation. Utilizes infection control and safety protocols appropriately.
