The Role

As a member of the Medicare Population Health team, the Quality Coordinator will report to the Senior Manager of Clinical Performance and Strategy and will work to support our quality strategy by targeting outreach to patients to close gaps, manage data from multiple payors as it relates to quality, implement medication adherence standard work and support operations to improve the health outcomes for our patients while reducing overall cost of care.

What you'll likely work on:
• Conduct targeted outreach to patients to close quality gaps for multiple measures, as informed by the quality strategy and market performance with specific targets.
• Implement targeted Medication Adherence programs for at-risk patients for multiple markets, including chart review and direct outreach.
• Data management from payor portals to support the CP&S Analysts.
• Manual gap closure through portals and application of annotations, suspects or subscribed marker sets to support quality initiatives.
• Critical thinking and problem solving when assigned ambiguous tasks with the ability to report out to stakeholders on findings in an effective, concise manner.
• Utilizing Excel, Google Sheets, and other analytic tools or software programs to extract, visualize and manipulate data.
• Shows a commitment to customer service by producing timely and accurate work for stakeholders.
• Prioritizing work and communicating with leadership when workload issues arise.
• Other duties, as assigned.

What you'll need:
• Minimum 1 year experience working as MSS, LSS, Health Coach or Operations Assistant.
• Previous experience making outbound calls to patients is required.
• Demonstrated track record of strong quantitative and analytical skills required.
• Knowledge of medicare quality measures is desired.
• Excellent organizational, written and verbal communication skills; experience with creating presentations in PowerPoint or Google Slides.
• Interest in learning how to interpret clinical data and its connec tion to day to day care.
• Able to work independently and as part of a team.
• Comfort with a high growth, fast paced, collaborative work environment.
