SUMMARY of DUTIES

OnTrack NY is a new, innovative treatment program that will serve adolescents and young adults (16-30 years old) who are within one year of experiencing the onset of non-affective psychosis. The Program Director is responsible for the overall management and supervision of the program, services, and staff. The Director will support recovery by providing services that reflect our deeply held values that include the recognition of individual rights of self-determination, choice, shared decision-making, and collaboration. When possible he/she will utilize evidence-based practices; always embracing a trauma-informed and person-centered approach.

SPECIFIC DUTIES and RESPONSIBILITIES:
• Provides overall supervision to the team and is responsible for the allocation of staff time.
• Ensures all team members are kept informed of issues involving participants and their families.
• Monitors caseload size for the team members.
• Coordinates program outreach, intakes, and discharges.
• Facilitates weekly team meetings and provides training and supervision for the IPS specialist and peer.
• Rotating on-call responsibilities with other team members.
• Models and teaches a person-centered approach, honoring choice, self-determination, and shared decision-making.
• Serves as the primary clinician for a subset of program participants.
• Follows the established agency policies and maintains appropriate professional standards.
• Responsible for adhering to all standards and guidelines established by applicable regulatory authorities.
• Develop and sustain collaboration with the internal team and external community providers.
• Establish and maintain a culture that is recovery-oriented and grounded in service excellence.
• Provide supervision, mentoring, coaching, and performance reviews for direct reports.

• The Program Director must have a valid Driver’s License.
• The Program Director must be cleared NYS Justice Center through fingerprinting for a Criminal History Records search.
• The Program Director must be cleared by the Justice Center for the Protection of People with Special Needs Staff Exclusion List (SEL) and the New York State Medicaid Exclusion List.
• The Program Director must be cleared by the NYS Office of Children and Family Services (OCFS) for instances of child abuse and/or neglect.

OnTrackNY is a team-based, innovative, coordinated specialty care program that provides services to young adolescents/adults experiencing their first psychotic episode. We provide recovery-oriented, evidence-based interventions to promote self-efficacy, resiliency, and overall functioning in the early stages of illness.

The OnTrackNY Peer Specialists will serve a unique role within the team. They work directly with participants and their families to make meaning of their experiences and support them to pursue their goals for school, work, and relationships. The Peer Specialist helps build an environment that conveys hope for recovery. Utilizing a shared decision-making approach, the Peer Specialist uses their lived experience and unique perspective to support, empower, and advocate for young people and their families on their recovery journeys.

Specific Duties and Responsibilities:
• Outreach participants and their families to introduce the program and its benefits to build a strong connection with the team. Promote community awareness and facilitate engagement with OnTrackNY.
• Develop authentic, meaningful relationships with individuals and families through empathy, sharing experiences, listening, and collaborating with genuine curiosity and interest.
• Explore and discuss multiple frameworks for understanding life experiences such as psychosis with participants and the rest of the OnTrackNY Team.
• Collaborate with participants to increase agency, self-awareness, and the building of life skills.
• Work with participants to define family and family involvement within the program.
• Support the growth and development of OnTrackNY through the creation and support of a feedback loop between participants and the team.

Our mission with the OnTrackNY Program is to empower young people to make meaning of their mental health experiences and to pursue their goals for school, work and personal relationships.

The IPS Employment Specialist develops competitive job opportunities in the community for individuals with mental illness. These jobs will be permanent in nature, competitive and pay at least minimum wage. They will be jobs that are available for anyone and not set aside for individuals with a disability. Will facilitate the successful utilization of job readiness training and job retention services for consumers of Vocational and PROS Programs. Building new relationships with potential employers in the community.

RESPONSIBILITIES
• Provide individualized support/coaching to consumers to assist them in achieving the outcome of employment and/or educational success
• Engages with community employers
• Assist consumers with resume writing, interview skills and job searching
• May transport consumers to and from interviews
• Assist/Coach consumers onsite at their place of employment
• Provide benefits counseling
• Provide consumers with accurate information regarding disclosure to employers
• Completes vocational assessment and updates consumer profile on a regular basis.
• Completes weekly and monthly performance indicators and records data
• Acts as a liaison between employer and consumer as needed
• Reports data to NYESS or to agency designee for reporting purposes
• Engaging participants and establishing trusting, collaborative relationships directed toward the goal of education or competitive employment in community job settings
• Collaborating with other team members to deliver the education & employment support within the context of the model (e.g., engaging with participants to do fun activities, practicing cognitive or social skills that can then help them get a job, etc.)
• Based on the participant's preferences, working closely with family members, or loved ones to educate them on how to support the young person with work/school goals
• Helping family or loved ones navigate employment and educational systems, identify resources that can be helpful to them, and/or providing other supports to families as needed to help the young person with their recovery.
• Providing job development and job search services directed toward positions that are individualized to the interests and uniqueness of the people on their situation and the situation of their support system, following the principles and procedures of the individual placement and support approach to supported employment.
• Conducting a minimum of 4 (IPS is 6) employer contacts per week.
• Provide support to the individual and the family for education goals in the community, including contact with staff at high schools, colleges, and other educational institutions
• Arranging access to entitlements counseling for the individual and/or family.
• Spending 65% or more of scheduled work hours in the community. For example, meeting participants at community locations (e.g. home, workplace, school, coffee shop, library, etc.); taking participants to apply for jobs; investigating local employment options; or meeting with employers regarding job development.
• Linking transitioning participants to services that can support work and school goals post-discharge from OnTrackNY.
• Performs all other duties as assigned.

QUALIFICATIONS
• Bachelor's degree in related field.
• Training and experience working with adolescents and/or young adults with serious mental illness preferred.
• Experience providing employment or education services preferred.
• Must have a working familiarity with community resources.
• Requires basic knowledge of psychiatric rehabilitation, vocational development, assessment, and placement techniques.
• Must have excellent communication, presentation, and networking skills as well as computer skills.
• Must be goal directed, organized and able to work independently.
• Valid NYS Driver's License.
• Must have accredited NYS Defensive Driving course; or obtain within 15 days of hire.
• Must maintain active NYS Defensive Driving course every 3 years, upon expiration.
