Job Summary:
CareLinx is looking for a Clinical Manager to assist with CareLinx’s Health Plan/Payer line of business. CareLinx is a healthcare technology platform that connects families with non-medical, in-home caregivers. This role will be supporting a large health plan whose members enroll in a non-medical home care benefit through CareLinx. The Clinical Manager will be responsible for monitoring and analyzing incoming data from tasks being completed in the home of members and ensuring the data is accurate and complete. Additionally, this role will help support members via educational outreach and occasional outreach to members’ primary care providers to help coordinate care.
As a Clinical Manager, you are the guiding expert on using data to identify clinical risks in the home for the health insurer to take early intervention steps. You need to have excellent communication skills and be comfortable working in ambiguity on occasion. You should also have experience in data analysis and be able to review data and identify gaps or potential interventions to impact positive health changes. There will be opportunities to affect program development through your clinical experience and to make recommendations to health plans and the rest of the CareLinx team from a clinical perspective. You should be a team player and be willing to learn about CareLinx’s health plan partners, including their expectations for their members. • Manage a caseload of families who will be enrolling in a non-medical home care benefit
• Monitor incoming care plan task data, which caregivers complete with members in the home and serve as a quality control to ensure the proper tasks are associated with the correct members
• Liaise with internal CareLinx teams to ensure caregivers are completing care plan tasks as intended
• Analyze and identify in-home care risks from data and escalate to the health plan as appropriate

Job Summary:
CareLinx is looking for a Clinical Manager to assist with CareLinx’s Health Plan/Payer line of business. CareLinx is a healthcare technology platform that connects families with non-medical, in-home caregivers. This role will be supporting a large health plan whose members enroll in a non-medical home care benefit through CareLinx. The Clinical Manager will be responsible for monitoring and analyzing incoming data from tasks being completed in the home of members and ensuring the data is accurate and complete. Job Summary
CareLinx by Sharecare and a large health plan are partnering to provide thousands of members with in-home care benefits and are looking for a caregiver in the state of Oregon. As an In-Home Caregiver you will be supporting elderly care by helping with daily activities and providing support to them in their homes and communities. You will play a pivotal role in helping with tasks such as light housekeeping, medication reminders, and other non-medical tasks. We are looking for experienced and interested caregivers ready to help as needed. Rate: $23-$28/hr. for CNA.
Essential Job Functions
• Meal preparation
• Housekeeping
• Medication reminders
• Personal care, such as bathing, dressing, and grooming
Qualifications
• Previous companionship and personal care experience
• Passionate about child & elderly care with a genuine desire to help others
• Reliable & professional
• Fully vaccinated for COVID-19 with one booster
• Trained in First Aid and CPR
• Trained in Emergency protocols and procedure
• Minimum of 6 to 1 year experience
