Our Hospice team has immediate openings for full time, part-time and PRN Admissions and Visits nurses available weekdays or weekends. You will work for one of the top Hospice programs in Montgomery county providing patients and their families with exceptional end-of-life care that supports dignity and comfort for individuals, peace of mind to families, and honors cultural traditions and customs.

Responsibilities include but not limited to:
• Perform initial assessment of patients' needs
• Obtain fully informed consents before admitting patients to the hospice program
• Serve as a clinical resource for referral sources to patients/family members
• Facilitate end of life conversations and decision-making with the family or caregiver.
• Prepare and discuss with physician an initial plan of care for patients
• Deliver quality hospice and palliative care services to patients in their place of residence
• Represent our agency well when communicating with patients, families, and physicians.
• Available schedules:
• Full-Time four, 10-hour, days during the week;
• Part-Time or PRN weekday hours are flexible;
• Weekends, must be available to work 8:30am-5pm every other Saturday and Sunday on the assigned weekend shift

Position Qualifications:
• An active RN license to practice in State of Maryland, CHPN certification preferred, and a minimum of 3 years of acute care and/or hospice experience.
• Demonstrated knowledge and skills necessary to provide meaningful care to communicate with the terminally ill.
• Excellent communication and customer service skills; attention to detail; strong organization skills, and proficient technical skills.
• Familiarity with Netsmart My-Unity electronic medical records a plus.
• A valid driver's license and reliable, independent transportation to provide care to hospice patients in Montgomery County.

Adventist Health Care seeks to hire a Registered Nurse (RN), within our Home Assistance Department, who will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing.

As a Home Assistance (AHA) Registered Nurse (RN) is responsible for:
• Direct on-site supervision of Certified Nursing Assistant (CNA) at least once every ninety days.
• Supervise Licensed Practical Nurse (LPN).
• Initiate plan of care for personal care clients.
• Make home visits with the purpose of creating Plans of Care as guidelines for the CMT/CMA-CNA in the safe and effective performance of their job duties.
• Make home visits with the purpose of creating Plans of Care and defining the medication administration plan as per the physician's orders and as guidelines for the aides in the safe and effective performance of their job duties.
• Continuous supervision of aide services as documented by nursing progress notes and CNA Supervision forms.
• Assist with training and education of CNA's by conducting in-services as well as orientation on occasion.
• Ensure the quality of aide services by conducting in home competency evaluations as needed.
• Care will include the management of medications, PCP follow up visits and weekly telephone calls for 60 days.

Qualifications include:
• Bachelor's degree in Nursing preferred
• 2 years' full time experience in Medical/Surgical or facility nursing within the past 5 years
• Valid Maryland RN licensure
• Valid American Heart Association BLS/CPR Certification
• Valid Driver's License with agency insurance requirements
