Basic Responsibilities for Job Title:
1. Provides excellent customer service at all times, to internal and external customers.
2. Assist the registered nurse (RN) to determine the participants’ needs for personal care and assistance in activities of daily living such as bathing, grooming, toileting and eating.
3. Document all skilled nursing care provided and orders in the patient’s chart on the day the service is given. Include quarterly signed and dated progress notes in each participant record.
4. Provide assistance to the participant in obtaining medical services from providers other than Adult Day Health Staff.
5. Monitor, administer and record prescribed medication.
6. Narcotic medication must be counted per two nurses at time of administration and must be counted daily by two nurses.
7. Assist Pharmacist consultant by providing updated medication profile for review of discrepancies.
8. Participate in medication profile review for each patient as scheduled by Nursing Supervisor.
9. Perform finger stick on participants with signs and symptoms of hyperglycemia while at ADHC as per MD order.
10. Observe LVN duties as assigned (i.e. patient transportation, oxygen tank check, bed linen washing, and medication cart inventory).
11. The LVN may be a member of the Assessment Team to assess the nursing needs of the participant.
12. Inform PMD/PCP or nutritionist of weight changes above or below parameters.
13. Provide health care education and counseling to the patient and family when indicated by the individualized plan of care.
14. Provide liaison to the patient’s personal physician, nursing, and other disciplines involved in the patient’s care.
15. Other duties as assigned by registered nurse or center Program Director.
