Summary: The Aging in Place Coordinator will provide information, referral and case management services to adults aged 55 and older and their families assisting them to live with dignity, respect, and independence in Boone County.

Essential duties and responsibilities:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Case Management
• Recruit and screen consumers per program guidelines
• Conduct intake and initial home assessment to identify safety concerns and needed services
• Assist consumers in identifying and setting goals for aging in place
• Monitor goal progression through consumer-driven regularly scheduled contact
• Assist consumers with service coordination and referrals for services as needed
• Refer consumers for in-house services for grocery shopping, food pantry delivery, friendly visiting, and lawn maintenance if needed
• Conduct annual re-assessment and evaluations
• Educate consumers about their rights and the independent living philosophy
• Assist consumers with the development of self-advocacy skills
• Assist consumers with reporting abuse, neglect, and exploitation
• Collaborate with the Education Coordinator on identifying programs to enrich older adults’ quality of life

Administrative
• Accurately enter case notes in the database and maintain documentation of case files pursuant to program regulations, internal policies, and external regulations
• Provide accurate and timely reports to management
• Assist with grant research and reporting
• Coordinate programs that promote independence such as Property Tax Credit Program and Holiday Basket Program
• Report suspected abuse, neglect, and/or exploitation of consumers to appropriate authorities

Outreach
• Conduct monthly tabling in the catchment area to raise community awareness of available services for un-served and under-served Seniors and their families.
• Collaborate and meet with other agencies serving Seniors to minimize service duplication
• Attend and represent the agency at internal and external events in the community

Qualifications:

Excellent written and verbal communication skills and highly organized. Comfortable working with diverse people, particularly with those with disabilities, seniors, and veterans. The applicant must pass a background check via the Family Care Safety Registry

Education and/or Experience:
• Bachelor’s degree or higher in psychology, social work, or human services field, preferred
• Minimum two years of experience working in social services, or 1 year working directly with seniors
• Knowledge in case management principles, procedures, and practices
• Knowledge of databases, dashboards, record keeping
• Knowledge of grant reporting
• Knowledge of community services and resources
• Appreciation of equity, inclusion, and accessibility for people of all abilities and the independent living philosophy

Other Skills and Ability:
• Valid Driver’s License and reliable transportation
• Ability to manage time effectively and maintain program integrity.
• Ability to present ideas effectively in oral and written format
• Ability to read, write, speak, and comprehend English fluently
• Ability to work with minimal supervision
• Ability to identify and solve complex problems and critically evaluate information for accuracy and usefulness in solving problems.
• Ability to use Microsoft products, databases, and other technology with comfort.
