Outreach
• Conduct monthly tabling in the catchment area to raise community awareness of available services for un-served and under-served Seniors and their families.
• Collaborate and meet with other agencies serving Seniors to minimize service duplication
• Attend and represent the agency at internal and external events in the community
Qualifications:
Excellent written and verbal communication skills and highly organized. Comfortable working with diverse people, particularly with those with disabilities, seniors, and veterans. The applicant must pass a background check via the Family Care Safety Registry
Education and/or Experience:
• Bachelor’s degree or higher in psychology, social work, or human services field, preferred
• Minimum two years of experience working in social services, or 1 year working directly with seniors
• Knowledge in case management principles, procedures, and practices
• Knowledge of databases, dashboards, record keeping
• Knowledge of grant reporting
• Knowledge of community services and resources

Summary...
The CNA/ Home Health Aide is a member of the home care team that is required to perform certain personal care and other unskilled tasks for patients in their home setting. The personal care services are necessary to meet the patient’s needs to promote a more comfortable and safe environment.
Job Duties: Each visit will be individualized so not all duties will be performed with each patient. This is not limited to these activities.
• Assist with patient ADLs including but not limited to bathing, dressing, grooming, shaving, eating, oral care activities, elimination, incontinence care, ambulation, skin, nail, and hair care, and placing clean linen on the patient’s bed.
• Companions will read conversate and listen to patient.
• Participate in recreational activities and hobbies.
• Accompanies patient on walks, trips, doctor’s visits, bank, grocery.
• Inform director of any change in assignment
• Light housework to maintain a clean, neat environment.
• May cook meals and assist patient with consumption of food.
• Encourages patient family to participate in care.
• Provides emotional support to family and patient.
• Assists in the maintenance of a safe and healthy environment.
• Uses equipment safely; cleans all equipment with appropriate cleaner after each use.
• Provides the patient with companionship, in many different levels, this includes taking patient shopping, to the doctor, on day trips.
• Reminds patient to take medication. Under NO circumstance is the HHA or companion to administer medication, if patient needs this service call director immediately.
• Takes a records vital signs (only if HHA, STNA or CNA.) once a day during initial visit.
• Advises agency director of change in condition.
• Documents all activities and findings. Submits documentation upon last visit weekly.
CNA and HHA will not perform any duties that may be viewed as practicing nursing duties. Especially administer medications, take physician’s orders or any procedures requiring the training, knowledge, or skill of a nurse specifically sterile techniques.
Qualifications:
To perform this job successfully one must be able to perform each job duty satisfactorily.
Education/Experience:
High School diploma or GED, completion of a state CNA, STNA, or HHA with a valid certification though the Ohio State Nurses Board.
State certification not necessary if only wishing to be a homemaker/companion. (Compensation is different.)
