Job Role Summary
The Chief Physician Executive (CPE) of Eskenazi Medical Group (EMG) and Eskenazi Health Center Inc. (EHC), leads, manages, directs and is accountable for all health center clinical services to assure health system’s goals are achieved. The CPE works to transform and sustain the practice as a Patient‐Centered Medical Home (PCMH). Within a PCMH, the CPE assures Primary Care services are high quality, multidisciplinary, comprehensive, coordinated, culturally sensitive, accessible, and delivered with preeminent service excellence. The CPE works to create an environment that is also compatible with and supportive of the teaching and research missions of the Indiana University School of Medicine (The School). The CPE is responsible for the appropriateness, quality, efficiency and effectiveness of the medical care delivered by EMG physicians and mid‐level providers within the health center. Representing EMG, and serving as a liaison to the health center staff, he/she works with the EMG CEO, the EHC CEO and CMO, the EH Service Chiefs, and IU School of Medicine education leadership to 1) develop and implement primary care services’ clinical policies and guidelines, 2) develop and assess clinical activities and performance, 3) monitor, develop, and attain EH fiscal and utilization objectives, 4) maintain and enhance the teaching environment, 5) enhance clinical research opportunities and 6) assure compliance with appropriate regulatory, accrediting and credentialing/privileging standards. Essential Functions and Responsibilities
Works closely with the EMG CEO (for the employment related issues below) and the Site Clinic Manager, EHC CMO, and EHC CEO for the practice issues below, the Chief Physician Executive:
1. Provides clinical oversight to lead, manage, direct and is accountable for all health center operations to ensure health system’s goals are achieved. The CPE does this in close collaboration with the Health Center’s Clinic Manager and Site Coordinator as a leadership team.
2. Oversees, directs, and ensures primary care physicians and advanced practice providers compliance with EH medical staff bylaws and all related policies and procedures. Conducts performance evaluations and feedback to primary care providers at his/her health center on an annual basis or more frequently if needed.
3. Provides input for the health center’s annual operating and capital budgets for approval by EHC Senior Leadership. The CPE, Health Center Manager, and Site Coordinator are together responsible for the site’s financial performance.
4. In collaboration with the Health Center Manager, determines care teams and staffing plans required to deliver care within site’s defined budget.
5. Participates in the recruitment, selection, and hiring of all primary care providers at the health center. Provides input in interviewing and the selection of the support staff. Provides oversight of shared service personnel assigned to the Health Center. Assists EHC leadership in preparing job descriptions and performing annual performance reviews for all providers at the site. Provides input into the annual performance reviews of the Clinic Manager, Site Coordinator and the support staff at the site. Works with EMG CEO and EMG Human Resources and with input from EHC CMO in determining appropriate probationary or other disciplinary action for providers. Provides input into appropriate probationary and disciplinary action for support staff.
6. With the Clinic Manager, develops the annual goals for the health center that addresses clinical quality, patient safety, patient satisfaction, service excellence, and the provision of cost effective clinical services; helps prioritize proposed programs and services and actively supports resulting strategies. Assures well-functioning Quality and Patient Safety teams at the health center.
7. Works with the EMG CEO, and EHC leadership to maintain and enhance primary care provider morale and a positive work environment at the CHC to actively promote the retention of primary care physicians and advanced practice providers. Works with EMG, EHC, EH, service chiefs and clinical department chairs to maintain sufficient faculty coverage for all teaching and other academic activities at his/her Community Health Center.
8. Within a matrix model of management the CPE shares oversight of the health center manager and site coordinator with the Director of Operations, oversight of the nursing staff with the Associate Chief Nursing Officer for Primary Care, and with the Director of Clinical Affairs for the medical assistants. The CPE works collaboratively with the EHC Director of Operations to fulfill administrative and clinical goals for health center.
9. Assists in identifying, developing and providing continuing professional education programs for clinical staff and provides mentoring, coaching and feedback for the primary care physicians and advanced practice providers. Promotes the integration of clinical practice, clinical education and research.

JOB SUMMARY:
The Newborn Nursery APRN will report to the Newborn Nursery Medical Director. Newborn Nursery APRN will provide clinical care for newborn patients admitted to the Eskenazi Hospital Newborn Nursery. The APRN plans, directs, and evaluates patient's health care at EMG venues. The APRN provides diagnostic as well as therapeutic intervention and evaluation, including diagnostic and pharmacologic intervention. Completes the scheduling function, registration, messaging, and/or admissions process
• Greets patients, guests, and family members.
• Schedules patients for services with an appropriate provider at appropriate locations and desired time when possible, ensuring accuracy and timeliness.
• Analyzes current patient information to determine if an account already exists so as not to duplicate records
• Creates an account for all patients who call for services or who present for services, including walk-in, non-scheduled, and emergency services according to the registration policy.
• Registers patients by entering accurate demographic, financial class, and insurance information; revises systems immediately as errors are recognized.
• Activates scheduled accounts that have been set up for the patient according to the registration policy
• Resolves work queue errors in an accurate and timely fashion
Ensures all required forms are completed and other paperwork/documents are gathered and accurate

• Ensures patient/guarantor sign all applicable documentation, such as consents and financial assistance application

• Performs insurance verification tasks, including running automated eligibility response at point-of-service to ensure active coverage and completing notification of admission with the insurance company within the established timeframe
• Completes messages for providers as needed using the In-Basket messaging system, ensuring that all information contained in the message is accurate.
• Updates Electronic Medical Record with documentation to communicate any information related to the status of a patient's account
Performs financial analysis of each case and informs the patient of financial responsibility
• Identifies patient copayment and reminds the patient of the collection process at the time of visit. When applicable, will inform patient/guarantor of liability due, including prior balances and estimates for scheduled service
• Attempts to collect payment at the point of service for both copayments and residual payments.
• Provides patient information on LCMC's financial assistance programs and/or refers patients to financial counselors as needed
• Maximizes point-of-service collection, meeting established registration collection goals
• Provides excellent customer service to all patients, guests, and family members and internal and external team

Job Role Summary
A Primary Care Physician of Eskenazi Medical Group (EMG) and Eskenazi Health Center Inc. (EHC), provides the full scope of services which fall under individual field of training, including but not limited to diagnosis, treatment, coordination of care, preventive care and health maintenance. In addition, a Primary Care Physician is a member of the Eskenazi Health Center team. In that capacity they are expected to be committed to the philosophy, goals and objectives of the Center and work collaboratively to achieve them.
It is the responsibility of the Primary Care Physician to model and uphold the mission, vision and standard of performance within the health center. The Primary Care Physician must be cognizant of, and comply with, all Center policies and procedures, EMG employment requirements and procedures as well as pertinent state and federal regulations.
Essential Functions and Responsibilities
Clinical Practice and Education
1. Obtains health history and perform examinations
2. Diagnoses and directs counseling for each patient on a plan for treatment

11. Actively supports and participates in all quality of care, performance improvement, and patient safety activities

17. When requested by leadership, represents EMG and/or EHC, and/or EH on assigned internal and external committees, task forces, commissions, agencies and promotional or public relations efforts regarding EH's primary care clinical affairs
18. Maintains flexibility (or availability) for the appropriate number of patient appointments in accordance with individual clinical FTEE. Is flexible to changing schedules to ensure access for essential patient needs

15. Accurately and consistently completes and attests notes from patient visits, signs and closes encounters preferably within 24 hours, but absolutely within 72 hours of the encounter
16. Meets the requirements and deadlines for all provider mandates from EH, EHC, and EMG, including but not necessarily limited to required CME hours, completion of annual PPD, annual flu shot, and any mandatory training assignments
17. When requested by leadership, represents EMG and/or EHC, and/or EH on assigned internal and external committees, task forces, commissions, agencies and promotional or public relations efforts regarding EH's primary care clinical affairs
18. Maintains flexibility (or availability) for the appropriate number of patient appointments in accordance with individual clinical FTEE. Is flexible to changing schedules to ensure access for essential patient needs
19. Works closely with the CPE or Medical Director for practice quality and performance requirements

Minimum Qualifications, Training, and Experience
1. Current licensure as an MD to practice in the State of Indiana by the State Board of Medical Examiners.
2. Current DEA, CSR Registration
3. Board Eligible or Boards Certified in physician specialty. If Board Eligible at the time of hire, must be Board Certified within 3 years of hire date
4. Insurability (malpractice)
5. Current BLS or PALS
6. Must be able to meet and maintain current Eskenazi Health credentialing and privileging requirements.
