Job Description:

Our Nurses work in collaborative environments with strong interdisciplinary teams, focused on a patient centered care model in a richly diverse work space. Under the direction and general supervision of the Nurse Manager, the registered nurse utilizes evidence based practices in accordance with the State Nurse Practice Act, policies and procedures of the hospital and as directed by the medical staff. The registered nurse who through knowledge and ability uses the nursing process to develop a plan of care which includes standards of practice to meet the physical, emotional spiritual, cultural and educational needs of the patient and family.

The Block Nurse is an expert in the nursing care of patients undergoing regional anesthesia procedures including knowledge of anesthetic agents and techniques. This nurse will partner with anesthesiologists and the perioperative team to provide excellence in pain management through strong procedural performance, critical thinking, advocation, and compassion.

Qualifications
• Current unrestricted Connecticut RN License.
• Graduate of an accredited nursing school (BSN preferred).
• Proof of Arrhythmia Certification required prior to date of hire.
• BLS certification by the American Heart Association prior to date of hire.
• ACLS certification by the American Heart Association prior to date of hire for external candidates and within 90 days for internal candidates.
• 1 year of recent Perioperative, Block Nurse, PACU, or ICU experience.

Under the general supervision of the Senior Director of Operational Coordination, Integration, and Excellence, the Operations Manager of Food4Health is responsible for the management of day-to-day operations and department personnel of the Food4Health (F4H) program. As a member of the F4H management team, assists in the development of department goals, objectives and standards, quality assurance, department education, and budget development, preparation, and control.

Coordinates department activities to include regulatory requirements, quality control and performance improvement. Responsible for hiring, supervising, and evaluating team members. Serves as an effective liaison between department staff, leaders, and other department leaders across Hartford Healthcare (HHC). Implements, executes, and reports on all aspects of the program while overseeing the community food pantry initiative.

Qualifications

Required:

Bachelor’s degree

5 years of leadership and management experience. Familiarity with the needs of Hartford’s inner-city community and public policy issues related to food availability.

Food pantry certification or ability to obtain this credential through Foodshare

Preferred:

Master’s degree in education, social services, or public policy

Demonstrated knowledge of adult education and group facilitation; experience with online learning and related platforms (Google Classroom, Zoom, etc.)

Bilingual English and Spanish.

Job Responsibilities
• Maintain ethical principles and professional standards of practice in adherence to NASW Code of Ethics
• Recognize and address health disparities-inequalities and injustices directed toward clients, related to access to care and provision of health services
• Develop and maintain cultural competence with an understanding of the history, traditions, values, and family systems of client groups as they relate to health care and decision-making. Display a sensitivity to and awareness of the diversity in cultural groups and integrate this knowledge into their practice
• Provide ongoing assessment, including gathering comprehensive information to use in developing interventions and treatment strategies.
• Implement intervention and treatment plans that promote client well-being and ensure a continuum of care. Planning shall be based on a comprehensive, culturally competent assessment with interdisciplinary input.
• Optimize client functioning through case management that facilitates collaboration among providers to address the client’s biomedical and psychosocial needs to better provide efficient, appropriate, and beneficial health care services to a client with multiple needs.

• Assume responsibility for continued professional development in accordance with the NASW Standards for Continuing Professional Education and state requirements
• Provide leadership to ensure access to care and to improve and maintain the quality of care provided demonstrated by contributions to teams and work groups across HH and HHC and include mentoring others within and outside the social work profession.
• Demonstrates age specific competence, including knowledge, behaviors and community resources, to meet patients’ needs of all age groups, neonates to geriatric populations; work collaboratively with age-specific hospital-based resources, including Geriatric specialists, pre- and post-natal specialists, and other clinical specialists.
• Active participant in Social Work H3W Huddles, Meetings, generating ideas and works toward solutions.
• Participant in the Social Work Council structure, embracing department QAPI process, tracks Social work dashboard and is aware of departmental metrics. Can describe the HHC balanced scorecard and cascading metrics for Patient Care Services and Social Work Services.

Qualifications
• Hold a master's degree from a social work program accredited by the Council on Social Work Education.
• Licensed Master Social Worker (LMSW).
• (LCSW should be acquired within 6 months of eligibility).
• Must be able to communicate effectively orally and in writing.
• Computer literacy.
