The Case Management Extender (PFK) is responsible for helping patients and their families to navigate and access community services and other resources, and for supporting and maintaining population health programs and care coordination activities. The Case Management Extender works with Partners for Kids staff, hospital staff, health plans, and community agencies to arrange for and/or connect patients to needed services. The Case Management Extender will identify, create and nurture relationships with local agencies, schools, churches and other programs that can provide services to children and their families. Case Management Extenders are patient and family facing and they interact with them on a daily basis. These activities are performed in a variety of settings including during inpatient stays, outpatient specialty clinic visits, emergency room visits, home and community visits and telephonically.

Responsibilities

The Case Management Extender (CMX) functions as a member of an interdisciplinary team and provides care coordination services to patients and their families including:
• Screening and identification of patients (Occasionally)
• Outreach to primary care physicians, specialty physicians and other service providers to identify patients in need of care coordination (Frequently)
• Assisting Care Coordinator with the development of family-centered plan of care (Occasionally)
• Facilitating communication and collaboration amongst the healthcare team (Frequently)
• Developing patient's/family self-management skills through education and resource provision (Continuously)
• Performs ad hoc tasks as directed by Care Coordinator (Frequently)
• Managing transitions between settings, caregivers and providers (Frequently)
• Monitoring and follow-up across the continuum of care (Continuously)
• Participates in orientation and continuing education of staff and students as appropriate (Frequently)

Qualifications

EDUCATION:

Bachelor's degree preferred, with background in health care, public health, or related clinical field a plus.

LICENSURE:

Valid driver's license, good driving record and proof of auto insurance required (required to do home visits in 34 county area)

CERTIFICATIONS:

Active BLS certification

SKILLS:

Working knowledge of Medicaid and other regulatory agency standards.

EXPERIENCE:

Preferred experience working in healthcare in a patient facing role. Driving of personal vehicle is required. Care is often provided in patient homes.

MINIMUM PHYSICAL REQUIREMENTS:
• Bend/twist; squat/kneel; climb stairs/ladder; reaching above shoulder; lifting/carrying 0-20lbs; pushing/pulling 0-25lbs; work enviornment may include contact with communicable diseases and/or pathogens/patient equipment/machinery/electricity: OCCASIONALLY - 1-33% of the time.
• Standing/walking; driving; vision: depth perception/color vision/peripheral vision - FREQUENTLY - 34-66% of the time.
• Sitting; flexing/extending of neck; repetitive hand/arm use; computer skills; audible speech/hearing acuity; seeing - far/near; problem solving/decision making/interpreting data - CONTINUOUSLY - 67-100% of the time.

• Oversees the daily operations of the Administrative Assistant staff including: human resource functions, mentoring and training, performance evaluations, formulating and implementing departmental policies, and assigning new or additional responsibilities.

• Administer, according to standardized directions, a variety of psychological tests including intellectual, achievement and neuropsychological tests to children and selected adults from infancy to adulthood
• Score psychological tests according to test manuals and provides scored test protocols to supervising psychologist
• Observe the behavior of patients during testing and reports these observations in writing to supervising psychologist
• Maintain good rapport with patients by interacting in an age-appropriate manner

Qualifications

KNOWLEDGE , SKILLS AND ABILITIES REQUIRED
• Bachelor's degree with evidence of knowledge of test administration required.
• A master degree in psychology or additional graduate course work is desirable. Must be able to interact effectively with children of all ages.

• Kneeling or stooping to administer test to infant up to 45 minutes
